What You Should Know About the DHSS HCY Program

The DHSS Healthy Children and Youth (HCY) Program offers service coordination and authorization for medically necessary in-home services for Missouri HealthNet recipients with special needs. In this program, you will receive service coordination through home visits and links to services and resource that help these you or your loved ones live safely at home.

Who Are Eligible for This Program?

• Be a resident of Missouri
• Be a newborn to 21 years old
• Must need medically necessary services
• Be enrolled in MO HealthNet

What Are Some of the Services Included in the Service Coordination?
You will be assisted by a Special Health Care Needs (SHCN) Public Health Nurse Service Coordinator.

• Care Planning
  Representatives will determine your or your loved one’s overall health needs and find the right home healthcare providers that can meet them. A plan of care will also be customized as a guide for the caregiver to ensure optimal care for the patient at home.
• Transition Planning
  If you or your loved one will transition from facility to home, there will be assistance in ensuring a smooth transition from hospital discharge to the implementation of their home health care in Ferguson, Missouri.
• Medically Necessary Services
  This may include private duty nursing or personal care aide services, whichever may be appropriate for you or your loved one’s needs.
